All Snead State student-athletes must complete and turn in the following items at the beginning of the year:

Student-Athlete Academic Forms
- Before student-athletes can register for classes, they must be with a faculty advisor and have the athletic advising form filled out. | Athletic Advising Form
- Each month, student-athletes will submit the athletic academic progress form to their instructors. The instructors will complete the form and return it to the student-athlete by the next class meeting. Students must turn in the completed form to their coach by the 30th of each month. | Academic Progress Form

- To file an accident claim, please complete the BMI insurance form. | BMI Insurance Form
